We offer online booking for your convenience, but if you prefer you can also call, email, or text us to book an appointment. If you have Medicare as your primary insurance, please select the Medicare option when booking your appointment. We operate out-of-network for all other insurance plans, so if you have questions on our rates you can see the details at the link below.

NOTE TO EXISTING PATIENTS: We have switched to a new scheduling system. Please email so we can enter you into our system that will still allow you to schedule and cancel your own appointments.
